FOR PATIENTS + FAMILIES

RECOVERY CHANGES LIFE
FOR THE WHOLE FAMILY.

Life after an acquired brain injury can bring new questions, new responsibilities and changes in everyday life. Mindfull Neuro Recovery brings rehabilitation, medical oversight and support together to help individuals and families navigate what comes next.

AFTER DISCHARGE

COMING HOME IS A MILESTONE. NOT THE FINISH LINE.

Leaving the hospital or inpatient rehabilitation is an important step forward, but recovery often continues long after discharge. Everyday life can reveal new challenges with movement, communication, thinking, independence, relationships and returning to meaningful roles.

Mindfull helps bridge that transition by continuing coordinated neurological rehabilitation in the community while the individual lives at home.

RECOVERY CONTINUES WHERE LIFE HAPPENS.

PART OF THE RECOVERY TEAM

YOU KNOW YOUR PERSON.
WE KNOW NEURO REHABILITATION.
WE WORK TOGETHER.

Families often see changes, challenges and progress that cannot be captured during a therapy session alone. That perspective matters. We work with families to understand priorities, reinforce progress and prepare for what comes next.

UNDERSTAND THE PLAN

Learn what the treatment team is working toward and how each discipline contributes to the individual's recovery.

STAY CONNECTED

Share observations, questions and changes that can help the team understand how recovery is translating into everyday life.

PREPARE FOR WHAT'S NEXT

Prepare for greater independence, community participation and meaningful roles as needs and abilities change.

COORDINATED SUPPORT

THE RIGHT CARE SHOULD MAKE
LIFE MORE MANAGEABLE —
NOT MORE COMPLICATED.

Brain injury recovery can involve far more than therapy appointments. Mindfull brings clinical care and support services together so patients and families have a coordinated team working toward the same goals.

FAMILY EDUCATION

Helping families better understand brain injury, recovery and the individual's evolving needs.

CASE MANAGEMENT

Coordinating services, follow-up needs and resources across the recovery journey.

CLINICAL COMMUNICATION

Keeping rehabilitation goals and medical needs connected through an interdisciplinary team.

COMMUNITY REINTEGRATION

Supporting the return to family roles, relationships, community activities, work or other meaningful pursuits when appropriate.

WHAT PROGRESS CAN MEAN

RECOVERY ISN'T ONLY WHAT HAPPENS IN THERAPY.

Progress becomes meaningful when it carries into everyday life. The goals are different for every person, but recovery may mean communicating more effectively, managing daily routines, participating in family life, returning to the community, preparing for work or school, or becoming more independent.

MORE FUNCTION.

MORE PARTICIPATION.

MORE LIFE.

YOU DON'T HAVE TO FIGURE OUT THE NEXT STEP ALONE.

Talk with our team about where your family member is in recovery and whether neurological day treatment may be an appropriate next step.
